Exclusion

  • Products (toothpaste, etc) teeth whitening, orthodontia and veneers are excluded
  • Cash discounts & senior discounts shall not apply
  • Care Credit and Cherry cannot be used to pay for the plan subscription or treatment.
  • Unless otherwise specified, plan discounts do not apply to services performed by a dental specialist.
  • Member savings and plan incentives are exclusive to this dental office.
  • Plan savings cannot be combined with any other offers or discounts.

Disclosures

  • This membership plan (the “Plan”) is not insurance. Plan members pay periodic membership fees in exchange for access to discounts on certain identified health care services rendered by participating providers in accordance with the plan fee schedule. Plan members are obligated to pay providers directly for services rendered. Plan details, retail fees and member savings may vary by plan.
  • Plan is an annual contract and includes an auto renewal feature. You can opt out of auto renewal at any time from the Member Portal.
  • You may cancel your Plan membership at any time. You will receive a full refund of membership fees paid to Members if (i) the cancellation request is received within the first 30 days of the current annual contract term and (ii) no discounted dental services have been provided to any member(s) during the current term.
